About the Geriatric Medicine Consultation Service
The Geriatric Medicine Consultation Service is a specialized, intensive, interprofessional clinical service that provides patient-and family-centred comprehensive geriatric assessments and treatment recommendations for older patients admitted to Mount Sinai Hospital for treatment of medical or surgical illnesses. The Service is comprised of Geriatricians and an interprofessional team including a Geriatrics Clinical Nurse Specialist, Social Worker, Pharmacist and Physiotherapist who work closely with medical, surgical and emergency department physicians and their teams to provide customized care plans for older patients with acute medical, surgical and mental health issues with a focus on restoring independence and helping older patients transition back to their communities. Patients requiring ongoing care after leaving the hospital may be referred to the Outpatient Geriatric Medicine Clinic or another appropriate service.
How to Get Referred
Patients are typically referred to the Geriatric Medicine Consultation Service by their treating physician or teams in either the Emergency Department or once hospitalized by their Medical or Surgical care team.
