Home Care Options
Home Care supports coordinated by Circle of Care can help older loved ones to live at home independently. Circle of Care’s interprofessional team, consisting of social workers, personal support workers and other allied health professionals, supports older patients through the integrated coordination of the right mix of home care services. The team works closely with older adults to understand their concerns, navigate the health care system and assist them in the best way possible to ensure their optimal health and independence at all times.
Palliative and End-of-life Care Supports
Our Palliative and End-of-Life Care Supports help older adults and other individuals spend their final days in the comfort of their own homes. Our Palliative and End-of-Life Care services are provided by specially trained staff and volunteers who support individuals and their families by offering visiting hospice services, personal care, homemaking and caregiver relief.
Personal Support and Respite Care
Circle of Care’s Personal Support and Respite Care services provide practical support to over 13,000 persons and their families across the Greater Toronto Area (GTA) each year where and when it is needed most to help our clients live at home. Highly-trained and caring Personal Support Workers (PSWs) provide personal care, homemaking, relief and respite to individuals and their caregivers to enable them to successfully manage their daily routines.
Supports for Caregivers
Our caregiver support services assist individuals and their families with complex situations and provide options to make appropriate decisions that also allow their caregivers to feel supported so they can also provide the best care. Our professional and highly-trained Social Workers routinely provide education, counselling care and practical support to individuals and their families.
Support for Individuals with Alzheimer’s and Other Dementias
Our Personal Support Workers and Social Workers provide support to persons living with dementia and their caregivers, so they can remain independent for as long as possible. Our Personal Support Workers are specially trained in providing dementia care at home, while our Social Workers can help persons with dementia access a variety of other supports like Respite Care and Day Centres that can provide supervised environments to participate in activities, enjoy meals and socialize with others.
Meal and Nutritional Supports
Circle of Care’s Meal and Nutritional Supports provide many ways for our clients to enjoy a healthy and balanced meal to enable more active and independent living. The supports offered through Circle of Care include Meals on Wheels, which caters to regular and special diets including kosher diets, as well as assistance with in-home meal preparation. We also help our clients access Diners Clubs and Day Centres for Seniors across the Greater Toronto Area that provide social settings in which to have a nutritious meal.
Community Transportation Services
Circle of Care offers low-cost Community Transportation Services to support older adults who are suffering from an illness or frailty to carry out everyday tasks that can be challenging for them, such as shopping or visiting a doctor. We help people get around with government subsidized door-to-door assisted transportation options designed for older adults that are reliable, safe and affordable.
