Sinai Health/UHN’s Healthy Ageing and Geriatrics Program provides comprehensive care to older adults and their families, evaluates the effectiveness of different models of care to improve patient and system outcomes, trains physicians and other health professionals in the most current and relevant practices of geriatrics, and advocates for best practices and policies that will improve the care of our ageing population.

Through partnerships with our patients, their families and internal and external health care providers, we have brought together geriatric specialists in medicine, nursing, psychiatry, therapy, pharmacy, social work, primary, home and community care to redefine how care can be best delivered for older adults and their families across the entire continuum of care.

Sinai Health/UHN’s award-winning Acute Care for Elders (ACE) Strategy was launched in 2010 to improve how care to older patients is delivered. The hospital, under this strategy, continues to develop, implement and link together a series of evidence-informed models of care and best practices to create a more integrated experience for older patients, their families, and caregivers across the continuum of care. We are proud that we are being supported by the Canadian Foundation for Healthcare Improvement to help others implement our ACE Strategy in communities across Canada and around the world.

We are committed to the development of a health care system that provides the right care, in the right place, at the right time. We provide interprofessional expertise, conduct research and develop innovative care models.  We educate ourselves and others, and collaborate with all partners to enable healthy ageing.

If you would like to support our mission, please consider making a donation of funds to help improve the care of our older patients.

WITH THANKS TO OUR DONORS

We owe our very existence to a group of passionate volunteers who raised the initial funds, much of it in nickels and dimes, to begin our mission as a health care organization. Every donation of an individual’s time or funds makes a difference to the work we do. We would like to specifically thank our donors for their past, ongoing and future support and specifically acknowledge those who have made significant financial gifts to our Healthy Ageing and Geriatrics Program since its inception.
