About Our Geriatric Medicine Clinics
Our Geriatric Medicine Clinics work in partnership with our patients’ primary care and other specialist providers to offer the specialist support of geriatricians who provide comprehensive assessments and care planning recommendations and services, links to other specialist, primary, home and community services and programs, as well as ongoing follow-up, support, health education, and counselling as needed to enable healthy and active living.
Your Appointment at Our Geriatric Medicine Clinics
The Main Reception for our Geriatric Medicine Clinics is located in Room 457, on the 4th Floor of Mount Sinai Hospital, 600 University Ave., Toronto, Ontario. Our Clinic Reception Office is open Monday to Friday from 9:00 a.m. to 5:00 p.m. Medical visits to our Clinics are by appointment only.
Upon our patients’ first visit to Mount Sinai Hospital, they will need to visit the Admitting/Patient Registration Office on the Main Floor of Mount Sinai Hospital at 600 University Ave., and present their OHIP health card to receive a Mount Sinai Hospital card. Patients will need this card to check in to their appointment and for any other future appointments and tests at Mount Sinai Hospital.
Our clinic focuses on providing holistic assessments and care that promotes the independence of our patients. For their first appointment, patients are provided a two-hour assessment, often involving a medical trainee, our geriatrics pharmacist and a geriatrician and are asked to arrive 15 minutes early to complete our New Patient Questionnaire if they haven’t done so ahead of time. Follow-up appointments tend to be shorter and only 30-45 minutes in length.
We suggest that patients bring all of their medications and supplements, the names and telephone numbers of all their physicians, their Mount Sinai Hospital and Ontario Health Cards and a responsible companion, such as a caregiver, family member, or friend – whenever possible – to their appointments. We further encourage our patients to wear loose fitting clothes in case they need to be examined.
