About The Toronto Rehab Falls Prevention Program
The University Health Network’s Toronto Rehab Falls Prevention Program promotes balance and stability among its patients through 12 weekly two-hour sessions, consisting of education and falls prevention exercises. The program includes geriatricians, physiotherapists, occupational therapists, nurses, pharmacists and dietitians. The program works with patients on their physical abilities, provides them with recommendations for lifestyle changes and discusses whether they need to modify their homes to ensure they are safe.
Your Appointment at the Toronto Rehab Falls Prevention Program
The Toronto Rehab Falls Prevention Program is located at Toronto Rehab – University Centre in the South Wing on the 1st floor.
At your first visit, you will be seen by a nurse, physiotherapist and doctor to identify why you have been falling. We work with you on your physical abilities, provide you with recommendations for lifestyle changes and discuss with you whether you need to modify your home to make sure you’re safe. In making assessments, we check your balance, mobility, muscle strength and if you have a fear of falling.
If appropriate, you will be enrolled in our 10-week program of group education and exercise sessions. If a patient has special needs, we will provide individual counselling.
How To Get Referred
To be referred to the Toronto Rehab’s Falls Prevention Program, please have your primary care provider or specialist physician complete the Referral Form and fax it to 416-597-7066 or send it to TRI-Eskerfax.DayHospital@uhn.ca.
