About The Toronto Rehab Geriatric Day Hospital
The University Health Network’s Toronto Rehab Geriatric Day Hospital is an outpatient rehabilitation program that helps older adults with multiple health problems be more active in the community. The Geriatric Day Hospital service consists of geriatricians, nurses, physiotherapists, occupational therapists, speech language pathologists and social workers. Through detailed health assessments, the Geriatric Day Hospital develops personalized treatment plans, offers health education, and connects patients with community services.
Your Appointment at the Toronto Rehab Geriatric Day Hospital
The Toronto Rehab Geriatric Day Hospital is located at the Toronto Rehab University Centre in the South Wing on the 2nd floor.
When you arrive, you will sign in with the receptionist. You will need your health card (OHIP card) to sign-in. If you do not have an OHIP card, please bring another form of government-issued photo ID, such as a driver’s license or passport.
You may be given a Measuring Health Equity Questionnaire to fill out. This form contains questions about your background. We collect this information to find out who we serve and what unique needs you may have. The form is voluntary and you can choose ‘prefer not to answer’ to any or all questions. However, the information you choose to give us will help us improve the quality of care for you and others. First appointments take longer than follow-up appointments. Your first appointment can take 2 hours or more. Follow-up appointments usually take 15 to 30 minutes.
How To Get Referred
To be referred to the Toronto Rehab Geriatric Day Hospital, please have your primary care provider or specialist physician complete the Referral Form and fax it to 416-597-7066 or send it to TRI-Eskerfax.DayHospital@uhn.ca.
