About The UHN Memory Clinic
The University Health Network’s Memory Clinic at the Krembil Neuroscience Centre at Toronto Western Hospital supports patients to manage, overcome and cope with cognitive changes, including disorders of memory, language, thinking, or personality. The clinic includes neurologists, geriatric psychiatrists, a geriatrician, a clinical nurse specialist, an occupational therapist and a social worker. The Memory Clinic Team provides the complete spectrum of care for patients who are affected by a memory disorder.
Your Appointment at the UHN Memory Clinic
The UHN Memory Clinic is located at the Toronto Western Hospital in the West Wing, 5th floor, in the Division of Neurology. The appointment usually lasts about 1 to 3 hours. The patient can be seen by several members of the team. Memory tests and physical and neurological (brain function) examinations are performed. The family member or friend who comes in with the patient may also meet with the doctors and other team members if it’s necessary. Follow-up appointments usually take from 20 minutes to an hour.

To be referred to the UHN Memory Clinic, please have your primary care provider or specialist physician complete the Referral Form and fax it to 416-603-6402.
