About our Temmy Latner Centre for Palliative Care
Sinai Health System’s Temmy Latner Centre for Palliative Care provides patients who are living with an advanced progressive illness with support for pain and symptom management and access to end-of-life care services. It also provides support to their caregivers and families.
Through its Home-Based Palliative Care Program, a team of Palliative Care Physicians provides specialized palliative care services to patients in their own homes across the Greater Toronto Area and are available 24 hours a day, seven days a week. The program coordinates necessary home care services in partnership with Local Health Integration Network to provide its patients with the additional support they require from nurses, therapists, personal support workers, spiritual leaders, grief counsellors and other health care professionals in their homes.

How to Get Referred
To be referred to our Temmy Latner Centre for Palliative Care’s Home-Based Palliative Care Program, please have your physician complete the Referral Form and fax it to 416-586-4804.
Currently, eligible patients must:
- Have a valid OHIP Card or be registered with another Canadian provincial or territorial health insurance plan
- Have palliative care needs
- Live in the Home-Based Palliative Care Program’s catchment area
