Sinai Health/UHN’s Healthy Ageing and Geriatrics Program’s Older People’s Education and Navigation Strategy to Support Healthy Ageing (OPENS) initiative seeks to provide reliable information to older adults, caregivers, families, and health care professionals on topics essential to healthy ageing. Health education resources developed through OPENS are organized in four series:

Series:
Promoting Health and Wellness
The Promoting Health and Wellness Series addresses common concerns in the health related to the ageing process. Its resources provide information to support older adults, their families, friends and caregivers to make simple lifestyle changes that can help with the prevention, identification and management of chronic health issues.

Community Supports and Services
The Community Supports and Services Series provides information on organizations and programs that support older adults to age in their homes and communities. Resources in this category includes information that can help older adults, their families and caregivers to meet their transportation, nutrition, mobility and other daily living needs.

Diversity and Ageing
The Diversity and Ageing Series highlights organizations that provide culturally appropriate and specific services to Toronto’s diverse population of older adults. Resources in this category provide information on local day programs, social services, older adult centres, housing and other services to enable older Torontonians to access supports in their preferred languages.

Planning Your Care
The Planning Your Care Series supports older adults to prepare for health care decisions that may arise in their later years. Its resources provide information on end-of-life care options, substitute decision makers, advance care planning and other considerations.
