About The Geriatric Psychiatry Clinics
Our Geriatric Psychiatry Clinics at Bridgepoint work in partnership with our patients’ primary care and other specialist providers to offer the specialist support of geriatric psychiatrists who provide comprehensive geriatric mental health assessments and care planning recommendations and services, links to other specialist, home and community mental health services and programs, as well as ongoing follow-up, support, health education, and counselling as needed to enable healthy and active living.
How To Get Referred
To be referred to our Clinics, please have your primary care provider or specialist physician complete the Referral Form and fax it to 416-461-2089.

About The Full Circle Psychogeriatric Case Management Program
The Full Circle Psychogeriatric Case Management Program is a community-based Geriatric Psychiatry Clinic offered in partnership with WoodGreen Community Services and Bridgepoint. The clinic provides an assessment from a Geriatric Psychiatrist, who helps to plan and coordinate services to meet the needs of the clinic’s older patients, as well as determine goals and expected outcomes. The program’s services may be delivered at WoodGreen offices at 815 Danforth Ave. or in the patient’s home if appropriate.

The Cyril & Dorothy, Joel & Jill Reitman Centre’s Carers Program
Sinai Health System’s Cyril & Dorothy, Joel & Jill Reitman Centre for Alzheimer’s Support and Training is a leader in the development of innovative models of care that support caregivers to feel more confident and able in their roles, which they have demonstrated helps improve their ability to support an individual with dementia to remain at home longer.
The Reitman Centre’s interprofessional team includes Geriatric Psychiatrists, Social Workers and Occupational Therapists who are able to provide individual- or group-based supports to caregivers.
The centre’s CARERS Program is a specially designed 8-week program that provides caregivers with the support and training they need to provide care to older family members with a diagnosis of Alzheimer’s or another form of dementia. The program allows caregivers to fully participate, since support and care can be provided for their loved ones with dementia at the same time. The Reitman Centre offers its CARERS Program at Bridgepoint to three groups per year.
How To Get Referred
To be referred to our CARERS Program for individualized caregiver counselling and support, please have your primary, home, community or specialist care provider complete the Referral Form and fax it to 416-586-3231.
Currently, to participate in the CARERS Program, eligible caregivers must:
- Have a family member with a diagnosis of dementia living at home in need of care
- Provide direct care for their family member
- Be willing to participate in hands-on training in a group setting

About The Geriatric Medicine Consultation Service
The Geriatric Medicine Consultation Service is a specialized, intensive clinical service that provides patient-and family-centred comprehensive geriatric assessments and treatment recommendations for older patients admitted to Bridgepoint Hospital for their post-acute and rehabilitation care needs. The Service is comprised of a Geriatrician who works closely with other medical and rehabilitation physicians and their teams to provide customized care plans with a focus on restoring independence and helping older patients transition back to their communities.
Patients requiring ongoing care after leaving the hospital may be referred to an Outpatient Geriatric Medicine Clinic or another appropriate service.
How to Get Referred
Patients are typically referred to the Geriatric Medicine Consultation Service by their treating physician once they have been hospitalized.
