Self-Referral Process
Feeling overwhelmed by the number of options for home and community support? Our specialists are trained to help people navigate the health care system by explaining the eligibility process, the role of various public agencies, and suggesting possible options to promote independent living at home.

Personal Support & Homemaking: Our qualified nurses work with highly trained and dependable Personal Support Workers (PSWs) to provide compassionate care, including: Personal care such as grooming, bathing, dressing, and toileting; and Homemaking, including meal preparation, light cleaning and shopping.
Hospice at Home: Circle of Care’s hospice/palliative care program, accredited by Hospice Palliative Care Ontario, helps to improve the quality of life for those living with a terminal or life-limiting illness, and provides support for family members by recognizing and addressing the need for psychological, social, and spiritual care. Our specially trained staff and volunteers are matched with clients and their families to provide support, information, and resources throughout the end-of-life journey. In addition to our non-denominational hospice program, we also offer a special program designed for the Jewish community in partnership with Jewish Family & Child.
Friendly Visiting & Phone Pals: Our trained volunteers provide companionship and social contact to older adults through weekly visits or phone calls. Volunteers and clients are carefully matched by common interests and language/culture. The special relationship that can develop between a client and a volunteer enables the client to have greater social interaction to maintain healthy, independent living at home.
Kosher Meals on Wheels: Our Kosher Meals on Wheels program is one of the largest meals on wheels programs in Canada, serving Toronto and the southern part of York Region for more than 50 years. Each year, we deliver more than 200,000 meals thanks to the help of over 200 friendly volunteers. Our delicious and nutritionally balanced kosher meals are prepared under the Kashruth Council of Canada (COR).
Transportation (iRIDE): iRide Plus, our government-subsidized transportation service, helps older adults or people with mobility issues get around safely and affordably. Our friendly, trained drivers will drop you off and pick you up in clean, comfortable vehicles.
Exercise & Falls Prevention Classes: We provide free exercise and falls prevention classes to older adults living in the community. The Falls Prevention program is led by an Occupational Therapist, and the Group Exercise Classes are led by trained fitness professionals. Classes are funded by the Central Local Health Integration Network (CLIN), and are offered to eligible participants free of charge.
Monthly Social Group: Our Let’s Get Together program offers monthly sessions for older adults to socialize, learn and enjoy lunch with great company. This program is available at various locations in Toronto and is available in both English and Russian. This program is ideal for those who are unable to get out on their own, or who have a limited family/social network.
Adult Day Program: For older adults living with dementias such as Alzheimer’s disease, the Adult Day Program is an opportunity to socialize and be stimulated within a secure and caring environment. The program is run by a compassionate team of specially educated social workers, therapeutic recreationists and program assistants.
Holocaust Survivor Support: Support for Holocaust survivors requires special understanding, knowledge and sensitivity. That is what we offer at Circle of Care. With oversight from specially trained staff, we offer a range of services for Holocaust survivors. These services are funded by the Conference on Jewish Material Claims Against Germany.
Elder Abuse Prevention & Intervention: Almost 1 in 10 older Canadians have suffered from elder abuse. Abuse can be physical, verbal, emotional, financial, or caused by neglect. If you or someone you care for is facing abuse, we provide: counselling, help in developing a plan to keep safe, and information and referral to legal, financial, housing and community resources.
Caregiver Respite: Being a caregiver can be overwhelming. As someone you love begins to depend more on you, you may find yourself caring for the day-to-day needs of your loved one. It can be very challenging to balance this with your other responsibilities. If it becomes overwhelming, we can provide help with respite care.
Client & Family Counselling: Our Social Workers offer emotional support and guidance to individuals dealing with grief, facing abuse or feeling depressed. We are also there to guide family caregivers through the challenges of caring for their loved one.
Life-altering circumstances, such as the death of a loved one, abuse, or financial insecurity can be overwhelming. If you or someone you care about needs immediate help, we can be there for you.
Bereavement Support: Losing someone you love is painful. After a significant loss, you may experience many emotions, such as shock, anger, and guilt. Sometimes it may feel like the sadness will never end. We have a number of ways to help you through this difficult transition, including support groups and counselling sessions.
Support Groups & Sessions: We provide group support for individuals and their families, to help guide our clients through challenging and difficult circumstances. Our support groups take place in various locations around North York. They are run by a social worker, and provide support and guidance free of charge to individuals dealing with grief, facing abuse or feeling depressed. We are also there to guide family caregivers through the challenges of caring for their loved one.
