About our GEM Nurses
Geriatric Emergency Management (GEM) Nurses support older patients to maintain their functional abilities when they enter the emergency department and connect them with appropriate supports once they transition back home. GEM nurses help reduce hospitalizations and create more durable transitions for their patients by linking them with a variety of primary, home and community care supports and outpatient specialist geriatric services. For those patients who require an inpatient stay, GEM nurses further facilitate their care by initiating appropriate care plans as soon as possible and ensuring that the inpatient geriatrics consultation services follow up on identified issues. Our GEM nurses help manage the care and improve the outcomes of older patients in the emergency department.
How to Get Referred
Eligible patients who may benefit from being seen by a GEM Nurse are referred by their ED physician or nursing colleagues. Some patients who are already involved in our Integrated Client Care Program (ICCP) and the TC-LHIN House Calls Program are automatically seen by the GEM Nurses during their regular working hours. Patients and families who would like to be referred to see the GEM Nurse can certainly request this of their treating ED physician or nurse.
