Under our ACE Strategy, Sinai Health has vastly improved the quality of care it delivers to its older patients and significantly decreased their lengths of stay and readmissions, enabling a greater proportion to return to their homes after an inpatient stay. The past few years have seen a growing number of organizations across Canada and beyond take notice of our innovative approach and look to transfer its success to their own settings.
With the support of the federally funded Canadian Foundation for Healthcare Improvement (CFHI) and Canadian Frailty Network (CFN), we are now being enabled to help other health care organizations across Canada and around the world implement elements of our ACE Strategy. With an inaugural $1-million investment announced in 2016 by the Federal Minister of Health, 17 hospitals in Canada, and one in Iceland, are being supported to implement 58 specific ACE Strategy models of care and care practices.
Our 12-month learning collaborative provides participating organizations with educational materials, coaching from Sinai Health frontline staff serving as faculty leads, and tools, along with funding support, to implement their desired elder-friendly models of care and care practices. Overall, the collaborative hopes these organizations will continue to implement additional models and care practices over time and support other neighbouring organizations to do the same as well.
