About Our Community Paramedicine Program
Our Community Paramedicine program is a proactive and collaborative model of community care for older adults who are frequent users of 911 services. With the support of our Healthy Ageing and Geriatrics Program, 14 paramedics working through Toronto Paramedic Services conduct proactive home visits where they provide targeted Comprehensive Geriatric Assessments as well as chronic disease management education to older adults living in the Greater Toronto Area who had three or more 911 calls in the previous month with the call determinant of falls, chest pain, short of breath or general illness, or four 911 calls in the previous month for any reason. In better understanding the often unmet needs of these individuals, our community paramedics also help refer and connect them to other primary, home and community care and specialist services that may better meet their needs.
How to Get Referred
Eligible patients are automatically screened based on their prior use of Toronto Paramedic Services using the following criteria.
Currently, eligible patients must:
- Be a resident of the City of Toronto
- Be identified by Toronto Paramedic Services 911 dispatch data as a high-risk caller due to making either:
- Three or more calls in the past month for breathing complaints, chest pain, falls or a general illness complaint
- Four or more calls for any reason in the past month
- Consent to participate in the program
Once a potential client has been identified, a 12-month trend analysis is completed to establish the likelihood of repeat 911 calls.
