About the SPRINT Senior Care House Calls Program
The SPRINT Senior Care House Calls Program is a mobile primary health care program for older adults living at home with physical, cognitive and social frailties who can no longer easily access office-based primary care services. It provides comprehensive primary medical care, as well as occupational therapy, physiotherapy, social work services and connections to local home care and community supports. This program is offered through a partnership between SPRINT Senior Care and VHA Home Healthcare, working in collaboration with the Sinai Health System.
The House Calls team includes family physicians, nurse practitioners, social workers, occupational therapists and a physiotherapist, supported by team coordinators. Patients in this program also have access to in-home support from a Sinai Health System Geriatrician, as well as geriatric psychiatrists.

How to Get Referred
To be referred to the SPRINT Senior Care House Calls Program, please have your primary, home, community or specialist care provider complete the Referral Form and fax it to 416-481-2590.
The intake process can also be completed over the phone by calling the House Calls Team Coordinator at 416-481-5099 ext. 331.
Currently, eligible patients must:
- Be 65 years or older
- Have difficulty accessing a family physician because of physical, cognitive or social frailty
- Have a valid OHIP card
- Transfer their primary care from their current primary care provider to a TC-LHIN House Calls Program primary care provider
- Live in the TC-LHIN House Calls Program catchment area—i.e., have a postal code that begins with one of the following: M4G, M4N, M4P, M4R, M4S, M4W, M4T, M4V, M4X, M4Y, M5B, M5G, M5M, M5N, M5P, M5R, M5S, M5T, M6E, M6H, M6C, M6G, M6P, M6R, M6S, M6N
- Not live in a retirement residence or long-term care or nursing home
- Not actively be in need of palliative care at the time of enrolment
- Consent to being a patient and taking part in a preliminary assessment
