Patients, Families and Caregivers
Sinai Health System’s Healthy Ageing and Geriatrics Program provides compassionate and quality care to older adults, their families, and caregivers. Our program uses an interprofessional, team-based approach to enable older adults to live independently for as long as possible in the community.
Through partnerships with our patients, their families and internal and external heath care providers, we have brought together geriatric specialists in medicine, nursing, psychiatry, therapy, pharmacy and social work, primary and community care among other fields, to redefine how care can be best delivered for older adults and their families across the entire continuum of care.
We have developed and implemented a series of evidence-informed innovative models of care and care practices to provide a more seamless and integrated experience of care for older patients. We are proud that in a short period of time we have received national and international attention for our ability to deliver significantly better patient outcomes.
We offer a number of programs and services to help keep our older adults active, independent and healthy members of our communities.
