Health care professionals working today have a remarkable opportunity to make a positive impact on the future of Canada’s ageing population. To meet that opportunity, Sinai Health/UHN’s Healthy Ageing and Geriatrics Program supports older Canadians to live longer, healthier and more fulfilling lives.
In 2010, recognizing an opportunity to better serve Canada’s ageing population, Mount Sinai Hospital became the first acute care academic health sciences centre to make geriatrics a core strategic priority. Through the development and implementation of our now internationally recognized Acute Care for Elders (ACE) Strategy, we put the needs, interests and priorities of older patients and their families at the core of what we do as an organization.
Through partnerships with patients, families and local primary and community care providers, our program combines the expertise of geriatric specialists in medicine, nursing, psychiatry, therapy, pharmacy and social work to redefine how care can be best delivered to older patients and their families. This approach delivers care that is better designed to meet the unique needs of our older patients, their families and caregivers and it provides them with the tools and knowledge that will support them to age as independently as possible.
Our program has grown tremendously since it was first launched and now accomplishes more than delivering excellent care. Sinai Health/UHN’s Healthy Ageing and Geriatrics Program provides care to older adults, trains physicians and other health professionals in the most current and relevant practices of geriatrics, implements and evaluates new and innovative ways of caring for older adults, their families and caregivers, and advocates for the system-wide adoption of best practices and policies that will improve the care of our ageing population.
As Canadians grow older and their health care needs evolve, Canada’s health care system will need to evolve as well. We are proud that our Healthy Ageing and Geriatrics Program at Sinai Health/UHN is at the forefront of leading and innovating in the care of older Canadians to enable healthy ageing.
